The State Bureau of Vital Statistics and Registration maintains birth records from 1878 to the present. Birth certificates are also available from Local Registrars in the municipality where the birth occurred. You can obtain a copy of a birth certificate directly from the Bureau of Vital Statistics online, by written request, or in person.
To obtain your baby's birth certificate by mail, please print out and complete a Vital Statistics Application Form. You must use this form or your application will not be processed. You must provide exact information that answers all questions and identifies the information as recorded on the vital record. As required by Governor's Executive Order #18 you must provide proof of your relationship to the person named on the record; you must also provide valid Acceptable Forms of Identification. Please send check, or money order made payable to Treasurer, State of New Jersey to pay the required fees.
A State or Local Registrar may issue a certified copy of a vital record only to persons who establish themselves as the subject of the vital record, the subject's parent, legal guardian or legal representative, spouse, child, grandchild or sibling if of legal age, to a State or Federal agency for official purposes or pursuant to a court order. A person who does not qualify will receive a "certification." This is a document that may be used for information purposes, but not for identification or legal purposes.
